Please use the forms on this page to submit information to the church.

Some forms you are able to complete online and submit. Other forms you must print out the form, complete and sign then upload and submit at the bottom of this page.

 

Building Use

Use this form to put your event on the Calendar even if it is not at The Harvest. This form must be turned in before your event can be promoted.

Childcare

Childcare is for Harvest UMC groups and events only and can be reserved through the "Building Use Request Form."
